A lot of women reach an ADHD evaluation in their thirties or forties, often after their own child is diagnosed and the description sounds uncomfortably familiar.
In short: ADHD in women is frequently missed because it more often looks like inattention, internal restlessness and exhaustion from compensating, rather than the visible hyperactivity seen in boys. The classic picture of ADHD — a boy who cannot stay in his seat — is the version that gets noticed, because it disrupts a classroom. Girls more often have the inattentive presentation: quiet, disorganised, daydreaming, struggling but not disruptive. Nobody sends a quiet child to be evaluated.
Girls also compensate earlier and harder. Lists, rigid routines, perfectionism, working twice as long as classmates, and a great deal of masking. The compensation works, right up until the demands outgrow it.
Because the exhaustion and worry are the visible part, women are frequently treated for anxiety or depression first — sometimes correctly, sometimes instead of the diagnosis underneath.
Many women report that focus and emotional regulation shift across the menstrual cycle, and around pregnancy, postpartum and perimenopause. Research in this area is still developing and firm conclusions are limited, but it is worth raising with your clinician rather than assuming you are imagining it — particularly if symptoms worsen at predictable points, or first became unmanageable after a major hormonal change.

Girls with the inattentive presentation are the ones schools rarely flag, because they are not causing trouble. If homework takes hours, if she is bright but her grades do not show it, or if she is anxious and exhausted by the effort of keeping up, an evaluation is reasonable — and the written report can support a request for school accommodations.
