Why an ADHD answer is hard to get in California

California has more psychiatrists than any other state and still has some of the longest waits in the country, because it also has the most people. In practice that means a referral, a screening questionnaire, and then a date months out — and in the meantime nobody will tell you plainly whether this is ADHD or something else.

The gap is worse if you need an evaluation rather than a refill. Many practices that advertise fast ADHD care are staffed by prescribers working from a questionnaire. A questionnaire cannot tell the difference between ADHD, an anxiety disorder, a sleep disorder, depression, or a thyroid problem — and those look alike from the outside.

The time-zone advantage, plainly

Our clinicians work Central Time from Texas. California is two hours behind. That means our first appointments of the day land at 6:00 AM Pacific, and our lunchtime is your mid-morning.

If you have ever tried to find a psychiatry appointment that does not cost you a half-day of work, this is the practical difference: you can be seen before the workday starts, from home, and be at your desk on time. Saturday appointments run in the Pacific morning as well.

The trade-off is the other end of the day. We do not run evening appointments in Pacific time, so if evenings are the only slot that works for you, we are probably not the right fit — and it is better to know that now than after a booking.

Care in Spanish, by a clinician not an interpreter

Close to forty percent of Californians are Hispanic or Latino, and a great many families would rather describe what is happening in Spanish than translate it. Our practice is based in Laredo, on the Texas border, and Spanish-language care is ordinary here rather than a special arrangement.

That matters more in psychiatry than in most specialties. Describing attention, memory, mood and shame in a second language flattens them. You lose the detail a clinician needs, and you end up with a thinner evaluation.

What the evaluation actually involves

  1. A conversation, not a form

    A psychiatrist takes your history: how attention, memory and impulsivity show up at work, at school and at home, how far back it goes, and what else could explain it — sleep, mood, anxiety, substances, medical causes.

  2. Rating scales as evidence, not as the verdict

    Standardised scales are part of the picture. They inform the diagnosis; they do not make it. For a child, we ask for input from school as well as home, because ADHD has to show up in more than one setting.

  3. A clear answer and a written report

    You are told whether this is ADHD, what else was considered and ruled out, and what the options are. You keep the report — for your records, for a school, or for another clinician.

  4. Treatment, if you want it

    If a diagnosis is made, we discuss the options. Medication is prescribed only when it is clinically appropriate and permitted by state and federal law, and never before it has been explained to you.

For California parents and students

If the evaluation is for a child, the report is written so a California school can use it. Schools here work through 504 plans and IEPs, and both processes want documentation of the diagnosis and its functional impact — not just a note that says ADHD.

For college students, campus disability services generally ask for the same thing: a diagnosis from a qualified clinician, and a description of how it affects academic work. Our report is written with that requirement in mind.

California ADHD evaluation FAQs

Do I have to be in California for the appointment?

Yes. You must be physically located in California at the time of the visit, because that is where the clinician's licence applies. It is where you are that matters, not where you live.

How early can I be seen?

Our clinicians work Central Time, so the first appointments of the day are 6:00 AM Pacific. Saturday appointments run in the Pacific morning. We do not offer evening appointments in Pacific time.

Is the evaluation done by a psychiatrist?

Yes. Evaluations are led by a psychiatrist — a medical doctor — who can consider and rule out the medical and psychiatric conditions that look like ADHD, and who can prescribe when that is appropriate.

Can I be seen in Spanish?

Yes. We provide care in Spanish as a matter of course, not through an interpreter arrangement. Tell our team when you call and we will schedule accordingly.

Will I get a report I can give to my child's school?

Yes. The written report describes the diagnosis and its functional impact, which is what a California school asks for when considering a 504 plan or an IEP. You keep it and can share it with whomever you choose.

Will I be prescribed medication?

Only if a diagnosis is made, it is clinically appropriate, and it is permitted by state and federal law. No prescription is guaranteed, and nothing is prescribed before it has been explained to you.
